PLAYER TRYOUT PARTICIPATION, LIABILITY WAIVER & PARENT/GUARDIAN CONSENT

IMPORTANT — PLEASE READ CAREFULLY. This agreement applies to your child's Red Tornado FC tryout, including assessment drills, warm-ups, soccer exercises and activities reasonably connected with the tryout. It includes a release of certain legal claims, including claims alleging negligence, to the extent permitted by applicable law.

1. Assumption of Risk *

I understand that soccer tryouts involve inherent risks, including falls, physical contact and collisions, muscle strains, sprains, fractures, other injuries, overexertion, illness, weather conditions, uneven or slippery surfaces, soccer equipment and the actions of other participants. These risks may result in serious injury, disability or death. I voluntarily accept the risks associated with my child's participation in this tryout and related assessment activities, subject to applicable law.

2. Release and Waiver of Liability *

To the extent legally permitted, I, on my own behalf, release and discharge RED TORNADO FC and its owners, directors, officers, administrators, coaches, assistants, employees, contractors, volunteers, representatives and agents from claims for injury, illness, death or property loss or damage arising from my child's participation in this tryout. This release is intended to include claims alleging negligence, including negligent supervision, instruction, assessment, organization or operation, and claims relating to facilities, equipment or playing surfaces, to the fullest extent permitted by applicable law. Where applicable, it is also intended to benefit the facility operator or host organization involved in the tryout. I understand this agreement does not waive any right or liability that cannot lawfully be waived, and that my signature does not automatically extinguish my child's independent legal rights.

3. Indemnification *

To the extent permitted by law, I agree to indemnify and hold harmless the released parties against third-party claims and reasonable legal expenses arising from my own negligent or wrongful acts or omissions in connection with the tryout, or from my material breach of this agreement. Any responsibility for my child's acts or omissions applies only to the extent I am legally responsible. Nothing in this section requires indemnification where prohibited by law.

4. Medical Authorization *

If my child is injured or has a medical emergency during the tryout, I authorize Red Tornado FC staff to contact emergency services and, where reasonably necessary, arrange emergency assessment or treatment if I or the emergency contact cannot be reached promptly. I understand Club staff may not be medical professionals. I agree to provide accurate emergency contact information and disclose relevant medical or allergy information needed for safe participation.

5. Conduct and Safety *

I understand that my child must follow reasonable safety instructions and directions from coaches and authorized personnel and treat other participants, staff and facilities respectfully. Unsafe, abusive, disruptive or discriminatory behavior may result in removal from the tryout. I understand that attending a tryout does not guarantee selection, team placement or a roster position.

6. Photo and Video Choice *

Select one option. This choice is separate from the liability waiver.

I GRANT PERMISSION: Red Tornado FC may photograph or record my child during the tryout and use identifiable images for Club-related promotional, educational or social media purposes, subject to applicable law. I DO NOT GRANT PERMISSION: I do not grant permission to use identifiable images or video of my child for promotional, marketing or social media purposes. This choice does not prevent necessary internal records or other uses permitted by law.

7. Fee and Tryout Arrangements

The indoor tryout fee is $45 CAD per player, payable in cash on the day. No online payment is collected by this form. Red Tornado FC will contact the parent/guardian to arrange the tryout date, time and location. Submission does not guarantee selection or a roster position.

8. Ontario Law and Limits

This agreement is governed by the laws of Ontario and the applicable laws of Canada. Any provision found invalid or unenforceable will be limited or severed as necessary. Nothing in this agreement excludes any right or liability that cannot legally be excluded.

9. Final Acknowledgment and Electronic Signature *

I confirm that I am the parent or legal guardian authorized to submit this form for the child named above. I have read and understood this tryout waiver, had the opportunity to ask questions or seek independent legal advice, and voluntarily accept its terms on my own behalf and to the extent legally authorized. I understand that entering my name and submitting this form is intended to be my electronic signature and acceptance to the extent recognized by applicable law.

Indoor tryout fee: $45 CAD — CASH ON THE DAY.
No online payment is required.
